The aim of the article is to summarise the main principles of evidence-based public health and to define in which particular ways it differs from evidence-based clinical medicine. The paradigm of evidence-based medicine has dominated clinical practice for 30 years. The article is a reminder that in addition to classic anti-epidemic measures there is a possibility of prevention of communicable (infectious) diseases (but also of non-communicable chronic diseases) that are specifically targetted by nutrition programs. Since ancient times, when the causes and nature of diseases were unknown (so even precisely targetted drugs, let alone antibiotics, were unavailable), the only way to prevent and treat diseases was to change nutrition. Money, health, social relationships or feeling addicted - these arguments for stopping smoking have certainly been heard and considered by smokers many times. Wasn't that enough to decide to quit smoking? So what about the environment? Quitting smoking because of the environment is not a bad idea at all. Tobacco devastates our lives by affecting the environment in which we live on a global scale. This is the theme of this year's World No Tobacco Day (WHO). The environment is being destroyed by tobacco growing, cigarette production and distribution, and the subsequent waste. This can also be a good reason to stop using cigarettes.

The war in the Ukraine, resulting from a brutal attack by Russia, has inspired the author to recollect a visit to Kiev in 1976 and negotiations with colleagues at the Ukrainian Institute of General and Environmental Hygiene about experiences with monitoring the health impacts of environmental pollution. A warm reception, hospitality, interest in professional issues and the atmosphere of well-being during the stay in Kiev sharply contrasts with the ongoing terrible war.
